Prime Care Coordination works to help those with intellectual and/or developmental disabilities find the services and resources they need to maintain their independence and achieve optimal health, wellness, and life goals.

At the heart of this mission is the care manager, who offers Health Home core services including:

• Comprehensive care management
• Care coordination and health promotion
• Comprehensive transitional care
• Individual and family support
• Referral to community and social support services
• Use of Health Information Technology to link services

With a pay range of $22 to $24.50 per hour, care managers offer complete support for each person, taking the time to really get to know them and their needs.
